All information will be treated confidentially. Each question should be fully and accurately answered. No action can be taken on this application until all questions have been answered. In reading and answering the questions, be aware that none of the questions are intended to imply illegal preferences or discrimination based upon non-job-related information.

  • When you are selected as a final candidate, a complete background check will be done by the Standing Rock Gaming Commission. Please pay attention to the question regarding law violations.
  • All information must be listed with no omissions:
Name: Your complete name including middle initial.
Address: Current mailing address, including street address, box #, etc.
Phone #: A telephone number must be listed. If you do not have a telephone, list a message number where you are sure to get your messages.
Position Applying For:

List the position that you are most interested in first.
List any other positions next.

(if applicable)		 List your enrollment number and the tribe you are enrolled in.
Education and Training:

List High School Diploma and/or G.E.D.
(you may be required to submit a copy of your high school diploma or G.E.D.)
List any colleges attended.
List any other skills, training and what machines or equipment you can operate.
Employment History: List all employment history for the last five years. Complete all information requested. (Employer's name and address, dates, title, salary, reason for leaving.)
References: Three references must be listed. Past employers or someone who knows you well, but is not a relative.
Resume: You may simply copy and paste your current resume from your word processing program.
Disclosure Statement: Please read each statement carefully before filling in your full name.
